II.1.5)Short description of the contract or purchase(s)Dorset County Council as host authority for the provision of Public Health functions in Dorset, is seeking tenders from external providers to deliver to individual Service Users a 12 week weight loss management support programme that incorporates the following approaches, as detailed in the contract specification: Healthy eating/weight loss; Behavioural change; Target and Support; Physical activity.
The programme is required to be delivered across various areas within the County of Dorset which includes all of the local authority areas of Dorset, Bournemouth and Poole.
The Contract shall be for a period of 2 years commencing 1.5.2014.
The value of activity is estimated at 203,700 GBP per annum.
The wider service consists of two parts; the Service User referral process from health professional to the Healthy Choices referral hub and the opportunity for an eligible Service User to access weight management support over a 12 week period.
