NHS Great Yarmouth and Waveney wish to commission a value for money integrated Tier 2 and Tier 3 weight management service which will collectively contribute towards Great Yarmouth and Waveney's obesity management and treatment care pathway. Delivery should be targeted in Great Yarmouth and Waveney's areas of greatest deprivation and/or highest obesity prevalence, and patient access to the pathway will be subject to clear and defined referral criteria guidance.
The service will be outcome based and will integrate Tier 2 and Tier 3 services by triaging through a single point of access including readiness for change assessment. The service will also provide pre-op and post-op support for patients having bariatric surgery. The successful Provider will also be required to integrate with existing commissioned weight management services.
In addition to weight management services, the Provider will train volunteers from local communities to become Community Food Literacy Trainers, who will then deliver basic cooking skills and food literacy courses to a targeted population within their community (those with poor health outcomes).
