The Council wishes to commission a cost effective Home from Hospital Transport, Settlement and Support service to support people with discharge from hospital and prevention of admission & readmission to hospital. The aim is to enable service users to return home safely in a timely, dignified manner and to regain their confidence after a period in hospital or a short stay in A&E; in a way that will help to promote their independence, reduce dependency on intensive home care packages, and reduce the likelihood of hospital readmission or long term care.

The budget for this service is approximately 240 000 GBP per annum.

The contract will be awarded initially for 1 year and 3 months with a possibility to extend for 1 further year, depending on performance and availability of funding.

Transfer of Undertakings (Protection of Employment) Regulations 2006 (TUPE) may apply to this contract.
