The company would provide fully trained non clinical care coordinators to take referrals for patients who are medically fit and clinically stable for discharge, but who need to choose a care or residential home before being discharged. The patient and their family would be supported through the process of choosing and moving to a suitable care home with a dedicated and personalized care co-ordinator to accelerate the process.
